Investigation findings: The U.S. Department of Labor's Occupational Safety and Health Administration cited M & M Manufacturing on Mark IV Parkway for repeatedly failing to protect workers from amputation hazards by failing to guard the point of operation on a metal press that shapes sheet metal into a circular collar. The company was previously cited for a lack of machine guarding at three different facilities between 2012 and 2015.
OSHA opened the inspection on Sept. 30 after receiving a complaint alleging an amputation injury. Reporting an amputation is part of the new record keeping requirements, and as of Jan. 1, 2015, a company is required to report the hospitalization of one or more employees, amputation, or loss of an eye, within 24 hours to OSHA.
