Investigation findings: The U.S. Department of Labor's Occupational Safety and Health Administration's Appleton Area Office cited the commercial laundry equipment manufacturer for one repeated, one serious and one other-than-serious safety violation.
OSHA initiated an inspection after receiving a report that a 50-year-old employee's right hand was crushed when he came in contact with operating parts of a folding machine. The injury resulted in the amputation of his right middle finger. Investigators found the machine's safety guard was not properly installed. The company was previously cited for this violation in 2011.
Alliance Laundry Systems also failed to report the injury to OSHA within 24 hours as required.
Quote: "This injury was easily preventable by properly installing machine safety mechanisms," said Robert Bonack, OSHA's area director in Appleton. "Each year hundreds of workers are injured on the job because employers fail to follow basic safety precautions."
