A recent investigation by the U.S. Department of Labor has concluded that inadequate safety training and improper storage practices were key factors in a fatal accident involving a forklift operator at a tire distribution facility in Elm Mott, Texas. The incident occurred in June 2024 at Mitchell Industrial Tire Co. Inc.'s distribution center.
The Occupational Safety and Health Administration (OSHA) conducted an inspection following the accident and identified significant safety violations that posed serious risks to employees. The forklift operator was fatally injured when struck by a falling pallet of industrial truck tires due to the failure of a three-tier rack storage system. Inspectors found that the vertical supports of the system were damaged, and horizontal beam locks were missing.
As a result, Mitchell Industrial Tire Co. Inc. has been cited for 12 violations, including insufficient training for forklift operators and improper storage of unbanded pallets exceeding 16 feet in height. The company faces proposed penalties totaling $288,299.
OSHA Area Director Timothy Minor emphasized the importance of proper training: "Neglecting to train forklift operators properly can lead to devastating consequences, as seen at Mitchell Industrial Tire." He urged compliance with safety standards to prevent workplace hazards.
Mitchell Industrial Tire Co., established in 1953, specializes in manufacturing solid tires for material handling industries and operates three distribution centers across the United States.
The company has 15 business days from receiving the citation to comply with OSHA's findings, request an informal conference with OSHA’s area director, or contest the decision before the Occupational Safety and Health Review Commission.
